Sample Report Summary

These are some pages as examples from our inspection reports used to document the inspection of Aerial Fire Apparatus.

We have different reports for different types of Trucks, including but not limited to. Telesqurts, Tillers, 3-5 section ladder trucks, 3-5 section platforms, Sutphen’s, Aerialscope’s, and Bronto sky lifts.

As you can see, we inspect all aspects of the apparatus including lighting, safety devices, under chassis and cab, above and beyond just looking at the aerial device itself. These reports can then assist the customer with routine maintenance checks on their aerial device as well.

The major things we look for are called requirements, which will usually involve an issue about S.O.S. or Safety Operational Structural issues. Per NFPA, these issues must be corrected for certification. We also find the smaller issues that may become bigger issues later down the road, these are called recommended items. These are issues that are noticed that should be fixed,  but are not required for certification.

Our report, depending on the style of the truck, ranges from 27-36 pages and is all-inclusive and accompanied by a written discrepancy report that explains what was found and where. If the aerial device passes the inspection the reports will be accompanied by a certification and a sticker for the truck stating the date of certification and when the certification will expire. If it does not pass, there will be a verification letter included that after repairs are made can be returned and then certification will be forwarded to you.
